Complete the form and click the "Submit" button. Your Maintenance Request Form should automatically attach to an e-mail. Click "Send" on the e-mail to finalize your submission. If an email does not open after you click the submit button, you may need to save the Maintenance Request Form to your computer and then open it in Adobe in order for the submit button to work.
